Participation in Special Olympics Tennessee requires athletes to register through the new Center of Excellence (COE) Portal.
Welcome to the New Athlete Registration Process
Introducing the new Special Olympics Registration Portal: the Center of Excellence (COE). This easy-to-use online platform is designed for Special Olympics athletes, coaches, parents/guardians, and volunteers to register quickly and efficiently while staying connected through real-time communication.
The COE will significantly improve the user experience when registering for Special Olympics Tennessee programming.
The Waiver, Release, and Medical Forms and Class A forms still need to be completed, but the COE streamlines the completion of these forms, provides real-time updates of approvals, and provides reminders when the forms will expire.
The COE portal can be accessed from any device, including iPhone, Android, tablet, and computer. Please complete online registration and also complete the medical form below.

Health History Forms
The new requirement eliminates a major barrier requiring a licensed medical practitioner’s physical examination and signature. At the same time, the new registration form benefits Programs by providing more current information about athletes across all categories and benefits athletes by providing the most current health information in support of their overall participation. Many of our athletes have changes in their health and medications annually and it is in their best interest, and ours, to have updated information.
A medical practitioner’s signature is required on the Medical Exam Form for all SOTN and SONA Regional Events, State, USA, and World Games. This includes any overnight travel outside the local program. This form must be completed and signed by a Licensed Medical Practitioner (for example, Physician, Registered Nurse Practitioner, or Physician Assistant) who is qualified to conduct physical examinations and prescribe medications.
